Definition of Terms

Transition - the formal process of cooperative planning that will assist students with to move from school into the adult world.

Community Developmental Disability Organization (CDDO) - single point of entry agency responsible for services and supports provided to children and adults with developmental disabilities within a specific catchment area. Butler County: Flinthills Services, Inc. Sedgwick County: COMCARE

Developmental Disability (DD) -

Guardianship - legally appointed oversight for an adult with an impairment who lacks the capacity to manage the essential requirements for his/her own physical health or safety.

Home and Community Based Services (HCBS) - Under the Social Security Act, certain statutory limitations were waived in order to give states, who have received approval from the Department of Health and Human Services, the approval for innovation in providing Home and Community Based Services to eligible people who would otherwise require institutionalization in a nursing facility, hospital, or intensive care facility for the mentally retarded (ICF/MR). Kansas has approval to operate six such HCBS waivers: Developmental Disability (DD), Physical Disability (PD), Severely Emotionally Disturbed (SED), Head Injury (HI), and Frail Elderly (FE).

Independent Living - a division of Kansas Department of Social Rehabilitation Services (SRS) that manages programs in independent living which offer an array of services for individuals requiring assistance with personal, nursing/medical, and social needs. These services allow individuals to remain in their own homes as an alternative to costly institutional care. Medicaid - Federal/state matching funds program that provides preventive, primary, and acute health services for low-income and HCBS-waivered individuals, children, families in a cost-effective manner.

Medicare - Federal health insurance program for people 65 and older, certain people with disabilities, and people with permanent kidney failure treated with dialysis or a transplant. Medicare has two parts: A - hospital insurance B - medical insurance.

Supplementary Security Income (SSI) - provides monthly income and Medicaid to severely disabled individuals.

Vocational Rehabilitation Services (VRS) - focus on helping people with disabilities become gainfully employed and self-reliant through individualized community based services, integration, and consumer choice. Services may include vocational assessment; counseling and guidance; physical and mental restoration; training; rehabilitation technology; job placement; supported employment; and transition planning services for students with severe disabilities.
